❗️From July 01, 2025, without up-to-date data, your profile or the profile of your child under 18 will be unverified. Please come to the appointment with a child’s birth certificate or ID card (regardless of age 0-18), the doctor will check the relevance of the data.
Also, notify your family doctor in time to change/replace the ID card.

⁉️ What should parents or legal guardians do?

👉🏻 provide the child’s birth certificate or other document confirming kinship;
👉🏻 provide the name and date of birth of the legal representative (mother, father, legal guardian);
👉🏻 then the doctor will send an SMS to your mobile number with a code that you should inform the doctor to change the data.

❗️If there is an error in the electronic medical record in the certificate series, the child’s identification number should be provided.

❗️Foreign birth certificates must have a certified translation into Ukrainian

❗️❗️❗️Your family doctor should be informed if:

✔️you changed your child’s birth certificate (received a duplicate)
✔️ received an identification code
✔️ received a new passport (ID card)
✔️ changed your or your child’s surname, name
✔️ changed your mobile number

🚫 Unverified children will NOT be able to receive the full range of medical services!
