Kindergarten or school certificate: what do parents need to know?

📌 July is the right time to prepare for the new school year in a calm and orderly manner.

In order for a child to attend school or kindergarten, it is necessary to undergo a preventive medical examination by a family doctor or pediatrician and obtain certificates of Forms No. 086/о and No. 063/о.

What parents need to know:
✅ 1. What certificates are needed?
🔹 Form No. 086/о – certificate of medical examination
🔹 Form No. 063/о – preventive vaccination card
🔸 In some cases, additional certificates from narrow specialists (neurologist, ophthalmologist, etc.)

✅ 2. What mandatory vaccinations should a child have?
According to the National Calendar, a child should be vaccinated against:
🔹 Tuberculosis
🔹 Polio
🔹 Measles, mumps, rubella
🔹 Diphtheria, tetanus, whooping cough
🔹 Hepatitis B
🔹 Haemophilus influenzae (Hib) (for children under 5 years of age)
If there are gaps in the vaccination schedule, the doctor will draw up an individual schedule.

📅 3. When to undergo a medical examination?

Make an appointment with your family doctor or pediatrician as early as July to avoid queues and do everything according to plan.

🔹 Check whether the child’s documents are up to date in the EHS (verification)! If the child is not verified, you need to update his or her data in the electronic system.
🔹 The doctor will tell you whether your child needs consultations of specialized doctors.
🔹 Get your child all the necessary vaccinations in advance.
